The Road to Health Toolkit is written for African Americans/ African Ancestry and Hispanics/ Latinos who are at risk for type 2 diabetes—but really it can appeal to any audience. It will help people in these communities learn how to prevent or delay type 2 diabetes. This Flipchart is one of the tools in the toolkit. It is designed to support community health workers (CHWs)/ promotores de salud who serve the African American/African Ancestry and Hispanic/Latino communities.
